Your Visit
Upon arriving, you will be greeted and asked to provide the items listed above. There may be additional important information to read and complete, based on the reason for your visit. We are committed to providing personal, quality care that is as convenient as possible.
Your wait time will be based on the number of patients being treated before you, and our triage (the degree of urgency based on the severity of the medical condition) of other patients. After check-in, our friendly staff will call your name, take you to a private exam room, and check your vital signs. When the provider sees you, they will discuss the reason for your visit, listen to your concerns, perform an examination, communicate their diagnosis, and provide a treatment plan that is right for you. If any further testing is necessary or recommended, they will advise you on your options.
Checkout
After your visit with our provider, you will be taken to the checkout area to receive a record of your visit and treatment discharge instructions.
Follow Up
After your visit, you will receive a follow-up phone call to check on your recovery, see if you have any questions, and update you if there was any follow-up needed after your visit.
Inportant Info
Children under age 18 must be accompanied by a parent or legal guardian. Any other accompanying adult must have a notarized statement authorizing treatment, signed by the parent or guardian.
